摘要 目的:观察癫癎患者自然睡眠时相与癎样放电的关系,探讨脑电Holter 24 h监测即俗 称的动态脑电图(AEEG)在癫癎的诊断及分型中的应用价值。方法:回顾分析761例癫癎患者24 h AEEG监测资料,对发作间期清醒与睡眠期脑电图癎样放电的发作频率进行比较。结果:761例患者中 检出癎样放电553例,睡眠期检出495例(检出率89.5%),觉醒期检出309例(55.9%),癎样放电主要 出现于NREM睡眠Ⅰ-Ⅱ期;93例患者AEEG监测后发作类型得到了修正。结论:睡眠期癎样放电检 出率明显高于觉醒期,癫癎患者睡眠期癎样放电主要出现于NREMⅠ-Ⅱ期,AEEG有助于癫癎的诊 断及分型。 Objective:To observe the relationship between the sleep phase and the epileptiform discharges and to explore the application value of AEEG monitoring in the diagnosis of epilepsy . Methods: Twenty-four hour EEG data of 761 patients with epilepsy was analyzed retrospectively , and epileptiform discharges during wakefulness and sleepness was compared with each other . Results: Epileptiform discharges were observed in 495 patients at a detection rate of 89. 5% during sleep and they were found in 309 patients at a detection rate of 55. 9% during wakefulness . The epileptiform discharges were mainly found in the Ⅰ-Ⅱ stages of NREM sleep . The seizure type of epilepsy was corrected in 93 patients after AEEG monitoring . Conclusion : The detection rate of epileptiform discharges during sleepness might be higher than that during wakefulness . The epileptiform discharges of patients with epilepsy might be found mainly in the Ⅰ-Ⅱ stages of NREM . AEEG monitoring might be helpful in the diagnosis and classification of epilepsy .
