鞍区肿瘤的显微手术治疗进展

Developments in treatment of sellar region tumors by microsurgical techniques

目的 探讨近年来在显微手术治疗鞍区肿瘤方面的进展。方法 通过检索和查阅近年来有关鞍区肿瘤显微手术治疗方面的文献,加以复习和综述。结果 鞍区肿瘤以垂体腺瘤、颅咽管瘤和脑膜瘤最为常见,因鞍区位置深在,解剖结构复杂,手术难度较大,术后容易发生并发症,但在显微神经外科技术条件下,可以最大程度地切除肿瘤,使并发症降至最低。结论 鞍区肿瘤以良性多见,采用显微神经外科技术切除肿瘤是根本治疗方法,手术入路的选择至关重要,术后并发症多数可以防治。

Objective To investigate the recent developments in treatment of sellar tumors by microsurgical techniques.Methods The literatures about surgical treatment of sellar tumors were searched and reviewed.Results The pathological types of tumors locating at sellar region included pituitary adenoma,meningioma,and craniopharyngioma.Although surgical removing of sellar region tumors was relatively difficult and post-operation complications may occur because of the anatomical complexity of sellar region,most of sellar region tumors can be ablated to satisfied degree by microsurgical techniques and the post-operation complications can be minimized.Conclusion Great majority of the sellar region tumors are pathologically benign and the dominant therapeutic method is surgical abscission,in which choosing appropriate approach is important.After operation many complications may occur, but most of them can be prevented and cured.
