顺铂化疗中肾毒性的监测

摘要 为探讨顺铂化疗中肾毒性的影响,对24例顺铂治疗的恶性肿瘤患者尿β_2微球蛋白(β_2-MG)进行监测,并以10例健康人尿β_2-MG为正常参考值。结果表明,大剂量组与小剂量组在化疗后同一时间内尿β_2-MG差异显著(P<0.01);小剂量Ⅰ组化疗后尿β_2-MG显著增加(P<0.05);小剂量Ⅱ组化疗后尿β_2-MG虽高于正常,但无明显差异(P>0.05);大剂量Ⅰ组化疗后尿β_2-MG增加非常显著(P<0.01);大剂量Ⅱ组化疗后尿β_2-MG增加,差异非常显著(P<0.01)。提示顺铂小剂量分次给药,大剂量水化及胃复安的应用可明显减低肾毒性。 To investigate the foxic effect of cis- platinum diaminodichloride on kindeys during chemotherapy urine β_2-MG was determined in 24 cancer patients on cis-patinum and in 10 healthy controls. The results showed that there was very significant difference contents in urine β_2-MG between large dosage group and small dosage group in the same time after chemotherapy(P<0. 01). β_2-MG increased in the first group of small dosage (P<0. 05). There was significant difference between the second group of small dosage and the controls (P>0. 05) though the former had a higher level. The first group of large dosage had a marked increase of β_2- MG in urine (P<0. 01). So did the second group of large dosage (P<0. 01). Kindey toxicity could be decreased by using small dosage cis-platinum, large puantity liquid and metoclopranlide.
