经胸断流术治疗儿童门静脉高压症术后再出血

The effect of esophageal and gastric devascularization through thorax for reoccurent esophageal variceal bleeding in children's portal hypertension patients

目的: 根据经胸食管胃底断流术治疗门静脉高压症术后再出血的体会, 探讨处理儿童门静脉高压症术后再出血的手术方法。方法: 经胸入路, 彻底离断下段食管、上部胃的血管, 行胃底折叠术, 防止胃食管返流。结果: 4例患儿术后 2周复查食管胃底钡餐透视, 见食道静脉曲张消失 3例, 1例明显改善, 经术后随访, 无再出血。结论: 经胸断流术近、远期止血效果好, 是治疗儿童门静脉高压症术后再出血的较好方法。

Objective:To improve the effect of reoperation for postoperative upper gastrointestinal rebleeding due to portal hypertension. Methods:The operative procedure revolved in lower part esophagus and upper gastric devascularization plus gastroplication. Results:No patients had newly developed esophageal varices in barium meal examination.There was no rebleedmg on follow-up of two months ~ four years.Conclusion:Lower part esophagus and upper gastric devascularization through thorax have good short and long term result for the treatment of upper gastrointestinal rebleeding due to portal hypertention.
