摘要
目的 探讨基质金属蛋白酶 - 7(MMP - 7)在大肠癌中的表达及临床意义。方法 应用免疫组织化学方法检测MMP - 7在 86例行治愈性切除大肠癌组织中的表达情况 ,并比较MMP- 7的表达与大肠癌临床病理参数间的关系。结果 MMP - 7蛋白质集中表达于肿瘤浸润先端癌细胞内 ,阳性率为 6 1.6 % (5 3 86 )。MMP - 7的高表达与浸润深度、淋巴管浸润、淋巴结转移、术后肝转移、预后不良之间呈现密切相关性 (P <0 .0 5 )。MMP - 7阳性表达病例的 5年生存率明显低于阴性病例的 5年生存率 (P <0 .0 5 )。多变量分析显示 ,淋巴结转移是术后肝转移的独立预测因子 (P=0 .0 2 2 8) ,MMP - 7阳性表达的P值为 0 .0 5 87,接近于 0 .0 5 ,是术后肝转移的重要因子。结论 MMP - 7是行治愈性切除大肠癌患者术后肝转移、预后不良的重要因子。
Objective To investigate the expression of matrix metalloproteinase-7 (MMP-7) in primary colorectal cancer and their relationship with clinicopathological parameters. Methods The expression of MMP-7 was immunohistochemically analyzed using formalin-fixed, paraffin-embedded specimens from 86 colorectal cancer patients who had undergone curative surgery. Results The cytoplasm of cancer cells at the invasive margin was diffusely stained for MMP-7 and positive expression rate of MMP-7 was 61.6%(53/86). Increased MMP-7 expression was significantly correlated with the depth of invasion, lymphatic invasion, lymph node metastasis, liver metastasis and poor prognosis (P<0.05 ). Compared with the cases of negative MMP-7 expression, the patients with positive MMP-7 expression had a significantly lower 5-year survival rate (P<0.05). In multivariate analysis, lymph node metastasis independently correlates with liver metastasis (P=0.022 8), and increased MMP-7 expression seems to be an important risk factor for liver metastasis ( P=0.058 7 ). Conclusion MMP-7 expression at the invasive margin could be an important marker in predicting liver metastasis and poor prognosis after surgical treatment in patients with colorectal cancer.
