常规与微创电视窥镜下大隐静脉制备 被引量:3

A comparative study on conventional and endoscope-assisted greater saphenous vein harvesting

摘要 目的 对比研究常规与内窥镜辅助下大隐静脉制备,探讨内窥镜辅助下大隐静脉制备在冠状动脉搭桥中的应用价值。方法 选取单纯冠状动脉搭桥患者 36 例,采用微创内窥镜辅助下进行静脉制备(微创组),并与同期采用传统方法行大隐静脉制备的 50 例(传统组)进行对照研究。结果 微创组术后腿部切口并发症发生率明显低于传统组。传统组切口感染、切口裂开和切口渗出的发生率(28%)明显高于微创组(2 8%)(P<0 .01)。传统组慢性下肢浮肿和腿部感觉异常的发生率(26%和38%)均明显高于微创组(5 .7%和16 .7%)(P<0. 05和P<0.01)。微创组住院时间也明显短于传统组(P<0 .01)。结论 内窥镜辅助下大隐静脉制备能明显减少手术后下肢切口并发症。 Objective To compare conventional with endoscope-assisted greater saphenous vein harvesting and to see if endoscope-assisted greater saphenous vein harvesting can reduce postoperative leg incision complications.Methods Minimally invasive endoscopic saphenous vein harsvesting technique was used in 36 isolated patients undergoing coronary artery bypass grafting(CABG) and compaired with 50 conventional saphenous vein harsvesting.Results The incidence of postoperative leg complications in minimally invasive greater saphenous vein harvesting group was 2.8%,which was remarkably lower than that in conventional group(28%).The hospital-stay time was much shorter in minimally invasive group than that in the convertional.Conclusion Endoscope-assisted saphenous vein harvesting is an effective technique in reducing postoperative leg incision complications.
