脱亚胺基重组鼠聚微丝蛋白抗体诊断类风湿关节炎的研究 被引量:6

The study of the diagnostic value of deiminated recombinant rat filaggrin antibodies in patients with rheumatoid arthritis

摘要 目的评价脱亚胺基重组大鼠聚微丝蛋白抗体(ArFA)在类风湿关节炎(RA)诊断中的应用价值。方法分别测定了190例RA患者和150例其他患者的ArFA,以及196例RA患者和207例其他患者的抗环瓜氨酸肽(CCP)抗体,并分析了两种抗体诊断RA的敏感性、特异性、阴性预测值与阳性预测值。结果ArFA的敏感性、特异性、阳性预测值、阴性预测值分别为77.9%、93.3%、93.7%、76.9%;而抗CCP抗体的敏感性、特异性、阳性预测值、阴性预测值分别为67.9%、94.7%、92.4%、75.7%;ArFA的敏感性高于抗CCP抗体(P<0.05)。结论ArFA可作为RA诊断手段的补充,其较高的敏感性源于结构上的脱亚胺基。 Objective To examine the diagnostic value of deiminated recombinant rat filaggrin antibodies (ArFA) in patients with rheumatoid arthritis (RA). Methods ArFA were detected in 190 patients with RA and 150 patients with other diseases and anti-cyclic citrullinated peptide (CCP) antibody were detected in 196 patients with RA and 207 patients with other diseases respectively. The sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of the two antibodies were compared. Results The sensitivity, specificity, PPV and NPV of ArFA were 77.9%, 93.3%, 93.7%, 76.9% respectively, while those of anti-CCP were 67.9%, 94.7%, 92.4%, 75.7% respectively. The sensitivity of ArFA was higher than that of anti-CCP (P<0.05). Conclusion ArFA can be used as a satisfactory marker for RA and its higher sensitivity is due to the deiminated structure.
