摘要
目的 :探讨改良骨盆三点截骨术治疗髋臼发育不良的疗效。方法 :应用改良骨盆三点截骨术矫治髋臼发育不良患者 2 7例。首先将耻骨上支截断和耻骨弓中点截骨 1～ 1.5cm后 ,在髋臼上方按Salter法截骨 ,以足够大的梯形髂骨块嵌入臼上 ,使髋臼撑开向前外及下方旋转 ,矫正Sharp角至 35°左右 ,恢复股骨头正常覆盖率 0 .75以上。结果 :2 7例中 2 2例术后随访 7～ 82个月 ,平均 2 2个月。参照Mckay评级法 ,疗效优者 17例、良 5例、差 0例。结论 :改良骨盆三点截骨术治疗髋臼发育不良 ,方法简单有效 。
Objective:To investigate the clinical effects of improved triple osteotomy of pelvis in treatment of acetabular dysplasia.Methods:27 cases were treated by modified triple osteotomy.First,the osteotomy was made at the pubic upper ramus and pubic arch,then Salter’s osteotomy was applied above the acetabulum.A free trapeziform iliac bone was inserted into the superior gap of acetabulum,to distract the acetabulum and make it rotate toward front-lateral and downward.The Sharp angle was adjusted about 35°,improving the cover rate of femoral head >0.75.Results:22 cases were followed-up for 7-82 months(22 monthson the average) after operation.According to Mckay’s score the results were excellent in 17 cases,good in 5 cases and poor in none.Conclusion:This modified operation for the treatment of acetabular dysplasia is simple and effective and half of the cases don’t need transfusion.Early treatment can avoid or postpone osteoarthritis.
