摘要
目的 报道犬Bachmann束 (Bachmann’sbundle)与心房颤动 (atrialfibrillation,房颤 )的关系及其关键消融部位。方法 对 6只犬的Bachmann束经心内膜和心外膜途径进行标测和消融,在上腔静脉和主动脉根部之间,左、右心耳中点处注射无水乙醇消融至Bachmann束电位消失,心外膜呈现右心房和左心房电位逐渐分离。消融Bachmann束前后用相同刺激方案诱发房颤,观察消融对房颤的影响。结果 成功消融阻断 6只犬的Bachmann束,Bachmann束电位消失。消融前房颤的诱发率为100% (6 /6),消融后 6只犬均不能诱发房颤。结论 Bachmann束对房颤的发生和维持具有重要作用,其关键消融部位在上腔静脉和主动脉根部之间,左、右心耳中点处,消融终点是Bachmann束电位消失。
Objective The aim of this study is to elucidate the role of Bachmann’s bundle(BB)in the development of canine atrial fibrillation(AF)and the crucial ablation site.Methods Mapping and ablation of BB were performed in 6 normal dogs from epicardium and endocardium.And BB was ablated with ethanol at the midpoint of left and right atrial appendage between the superior vena cava and the aortic root until BB potential disappeared and right and left atrial potential were separated.The same programmed stimulation at different sites were carried out before and after ablation.The BB influence on AF after ablation was observed.Results All of the BB potential disappeared by ablation with ethanol in 6 dogs.AF was induced in all dogs before ablation and could not be induced after ablation.Conclusion BB plays an essential role in the development of the canine AF and the crucial ablation site may be at the midpoint of left and right atrial appendage between the superior vena cava and the aortic root.The end point of ablation is that BB potential disappears.
