城市社区高血压双向转诊路径的应用评价

Application and Evaluation of Clinical Two-way Referral Pathway for Patients with Hypertension in Urban Community

目的 评价所建立的适合我国城市社区人群的高血压双向转诊路径的效果、效益、可行性和可推广性。方法 采用非随机同期对照试验和个人深入访谈法, 对所建立的双向转诊路径进行评价。结果 实施高血压双向转诊路径可以缩短患者的就诊时间、降低再就诊率、节约医疗费用、提高患者的满意度、提高治疗效果并使服务具有连续性。结论 高血压双向转诊路径确实是一个具有成本效益的疾病管理模式。

Objective To evaluate the effectiveness,benefit,feasibility and extrapolability of the clinical two-way referral pathway developed in urban community for patients with hypertension.Methods The clinical two-way referral pathway developed in urban community was evaluated by non-random concurrent controlled trial and individual depth interview.Results The clinical two-way referral pathway for hypertension shortened the delayed admission time,decreased the readmission rate,reduced the medical cost,enhanced the satisfaction,improved the treatment effect and kept the continuity of the service.Conclusion The clinical two-way referral pathway for patients with hypertension may be a cost-benefit model for disease management.
