胰腺损伤的早期诊断与治疗

Early diagnosis and treatment of pancreatic injury

目的探讨胰腺外伤的早期诊断与治疗。方法对7例胰腺损伤中的3例轻度挫伤行保守治疗,2例胰源性腹水行胰周、腹腔引流术, 1例胰腺挫裂伤行缝合修补术, 1例胰体尾部严重挫裂伤行胰体尾部切除术。结果3例保守治疗、4例手术治疗均治愈。结论胰腺外伤后上腹部疼痛、腹水、血清淀粉酶升高,结合胰腺B超、CT检查可早期诊断,根据胰腺损伤情况可采用不同的治疗方法。

Objective:To investigate the experience of early diagnosis and treatment of pancreatic injury.Mehtods:Among seven cases of pancreatic injury,three cases of slight contusion were performed with a conservative treatment,two cases of pancreatogenic ascites with a drainage of pancreatic periphery or abdominal cavity,one case of contusion and laceration with suture and neoplasty,one case of severe contusion and laceration of pancereatic body and tail with resection of pancreatic body and tail.Results:Three cases of conservative treatment and four cases of operative treatment wre all cured.Conclusion:The early diagnosis can be made by epigastralgia,ascites,high serum amylase combined with B-mode ulˉtrasound and CTafter pancreatic injury.The different curative methods can be adopted according to the conditions of pancreatic injury.
