血小板膜糖蛋白Ⅱb/Ⅲa受体在房颤患者中的检测及临床意义

Detection and clinical significance of platelet membrane glycoproteinⅡb/Ⅲa expression in atrial fibrillation patients

目的 :研究心房颤动 (房颤 )患者中血小板膜糖蛋白Ⅱb/Ⅲa受体的变化及其临床意义。方法 :采用全血法流式细胞术分别测定房颤患者 (16例 )、非房颤窦性心律患者以下称非房颤患者 (2 8例 )及正常对照组 (17例 )血小板膜上活化糖蛋白Ⅱb/Ⅲa复合物水平 ,比较房颤患者与非房颤患者及正常对照组血小板膜糖蛋白Ⅱb/Ⅲa受体变化情况。结果 :房颤患者GPⅡb/Ⅲa测得值显著高于非房颤患者 (P <0 .0 5 )及正常对照组 (P <0 .0 5 ) ;非房颤患者GPⅡb/Ⅲa也显著高于正常对照组 (P <0 .0 5 )。结论 :房颤患者处于血栓前状态 ,引起血小板激活 ,GPⅡb/Ⅲa高表达可能与房颤患者易发生血栓栓塞并发症有关 ,检测GPⅡb/Ⅲa对评估房颤患者的血栓栓塞危险性及指导抗凝、减少栓塞并发症有一定的临床意义。

Objective To study the variation and clinical significance of platelet membrane glycoprotein Ⅱb/Ⅲa(GPⅡb/Ⅲa) receptor in atrial fibrillation patients.Methods Using flow cytometry, the whole blood GPⅡb/Ⅲa expression levels were respectively detected in 16 atrial fibrillation patients(atrial fibrillation group) and 28 patients with sinus rhythm non-atrial fibrillation (non-atrial fibrillation group) and 17 healthy controls(healthy group). The alterations were analysed in three groups.Results Compared with the non-atrial fibrillation group and the healthy control group, the expression of GPⅡb/Ⅲa in patients with atrial fibrillation was significantly higher (P<0.05); The expression levels of GPⅡb/Ⅲ a in patients with non-atrial fibrillation was also significantly higher than that in healthy control group (P<0.05). Conclusion Patients with atrial fibrillation are in prethrombotic state and platelet is activated in those patients.Higher GPⅡb/Ⅲ a expression levels may be correlated with the atrial thrombus and thromboembolism complication. To detect the presence of GPⅡb/Ⅲa may be helpful in evaluating embolic risks and guiding anticoagulation for reducing embolic events in atrial fibrillation patients.
