摘要
目的探讨白细胞介素7(IL-7)对肺结核患者外周血单个核细胞(PBMC)分泌干扰素γ(IFN-γ)、肿瘤坏死因子α(TNF-α)、白细胞介素4(IL-4)、白细胞介素10(IL-10)的影响。方法随机将20例肺结核患者和15名健康人PBMC分为RPMI1640组、PPD组、PPD+IL-7组、PPD+anti-IL-7组,培养72小时后收集上清液,采用酶联免疫吸附法(ELISA)检测各组培养上清液中IFN-γ、TNF-α、IL-4、IL-10的水平。结果与PPD组相比,加入IL-7能有效促进肺结核患者及健康人PBMC分泌IFN-γ(108.6±62.0-156.8±78.6,201.2±60.4-290.3±108.4)和TNF-α(463.5±172.2-880.9±249.3,1163.0±381.6-1716.8±522.6),抑制IL-4(58.8±14.7-36.9±13.2,47.8±17.7-35.3±11.8)和IL-10(154.4±45.9-111.0±34.4,136.9±46.4-99.3±45.7)的合成;而加入anti-IL-7可抑制IFN-γ和TNF-α的分泌,促进IL-4和IL-10的合成。肺结核患者各组IFN-γ、TNF-α水平均低于健康人各对照组(P<0.05)。而IL-4、IL-10差异无显著性意义(P>0.05)。Ⅱ型肺结核患者组IFN-γ、TNF-α水平低于Ⅲ型肺结核患者组(P<0.05),IL-4、IL-10水平高于Ⅲ型肺结核患者组(P<0.05)。结论IL-7通过诱导IFN-γ、TNF-α分泌,抑制IL-4、IL-10合成,从而调节Th1/Th2平衡。
Objective To investigate the effect of interleukin-7 on the production of interferon-γ (IFN-γ), tumor necrosis factor-α(TNF-α),interferon-4(IL-4) and interferon-10(lL-10) by peripheral blood mononuclear cells (PBMC) from patients with tuberculosis. Methods PBMCs were isolated and cultured from 20 patients with pulmonary tuberculosis and 15 healthy tuberculin reactors respectively. Every sample was divided by RPMI1640, PPD, PPD+IL-7, PPD+anti-IL-7. After 72 hours, the su-pernatants of culture PBMC were collected. The levels of IFN-γ, TNF-α, IL-4 and IL-10 were measured by ELISA. Results Compared with the PPD group,IL-7 increased the production of IFN-γ and TNF-α,but decreased the production of IL-4 and EL-10 in the patients and the healthy individuals. The levels of IFN-γ, TNF-α were lower in tuberculosis patients than those in the healthy controls (P<0. 05). The levels of IFN-γ and TNF-α were lower in disseminated tuberculosis patients than those in exudative tuberculosis cases (P<0. 05). The levels of IL-4 and IL-10 were higher in disseminated tuberculosis patients than those in exudative tuberculosis cases (P<0. 05). Conclusion Il-7 could regulate the balance between Th1 and Th2 cytokines by inducing IFN-γ, TNF-α production and inhibiting IL-4, IL-10 expression, which suggested IL-7 enhance the patient defense against the infection of tuberculosis.
