摘要
目的:探讨胃癌中黏蛋白1(MUC1)异常高表达与胃癌临床分期、组织学类型、肿瘤发生部位、肿瘤大小和细胞增殖活性等临床病理学关系。方法:收集手术切除胃癌标本61例,应用HE染色切片进行组织病理学分类与核分裂指数评价,EnVision两步法行MUC1与Ki-67单克隆抗体免疫组织化学染色,10%以上肿瘤细胞出现MUC1胞质着色视为高表达;40%以上肿瘤细胞核出现Ki-67阳性视为Ki-67标记指数增高。结果:32例胃癌(52.5%)有MUC1高表达,MUC1高表达组核分裂指数明显高于MUC1阴性组(P=0.027);胃底贲门部胃癌MUC1高表达病例较胃体幽门部胃癌相对多见(P=0.068);MUCI高表达与否与Ki-67标记指数高低一致率达81.9%。Ki-67标记指数与核分裂指数一致率可达75.4%。结论:MUC1高表达与胃癌细胞增殖活性增高有关,可将其作为反映胃癌增殖活性的有用标记物。
Objective To study the relationship between the overexpression of mucin 1(MUC1) and several clinicopathological parameters including histological classification, TNM stage, location, tumor size and mitosis index in gastric cancer. Methods A total of 61 samples of gastric cancer were collected. The high sensitive EnVision two steps immunohistochemical method was used to detect the aberrant expression of MUC1 and Ki-67. Over 10% cancer cells with cytoplasmic staining of MUC1 was considered as overexpression. Over 40% cancer cells with nuclear staining of Ki-67 was defined as high proliferative index of Ki-67. Mitosis index and histological classification were evaluated based on H&E staining. Results Thirty-two samples (52.5%) showed MUC1 overexpression. The gastric cancer cells with MUC1 overexpression had higher mitosis index compared with those in the non- MUC1 expression group(P=0.027). The tumor had the trend of upper-stomach location (P=0.068). The concordance rate of the MUC1 expression with the Ki-67 proliferative index was 81.9%. The concordance rate of the Ki-67 proliferative index with the mitotic index was 75.4%. Conclusions MUC1 involves in the proliferative regulation of the gastric carcinoma, which can be used as a marker to evaluate the tumor proliferative activity.
