三维超声在肾综合征出血热诊断中的应用

摘要 目的 对肾综合征出血热 (HFRS)伴有急性肾功能不全病程中肾锥体行三维立体成像 ,以评价其临床应用价值。方法 采用三维动态成像检测 4 5例经血清学证实 HFRS抗体阳性患者伴有急性肾功能衰竭及 30例无肾功不全者行双肾三维立体成像。结果 无肾功不全者肾锥体未能获得满意三维立体图像 ,而伴有急性肾功能衰竭的 HFRS通过三维重建后 ,肾锥体扩大 ,清晰、可见 ,呈蜂窝样改变 ,并对肾皮质产生压迹。结论 三维超声显像能直观形象地显示了扩大的肾锥体。 Objective To evaluate the clinical applicable value of three-dimensional ultrasound (3-DUS)for detecting renal pyramid lesions in patients with hemorrhagic fever renal syndrome (HFRS)with complicated acute renal insufficiency.Methods The bilateral kidneys in 45 patients with HFRS and with positive HFRS antibody complicated with acute renal insufficiency(ARIS) and in thirty patients without ARIS as the control (C) group were detected using 3-DUS.Results No satisfactory 3-D stereoscopic images were found in the renal pyramids of the c group,whereas the images of HFRS patients were clear and showed enlarged and alveolate renal pyramids with renal cortex compression marks.Conclusions 3-D US can visually demonstrate abnormal renal pyramids which are the diagnostic basis for HFRS with ARIS.
