高强度聚焦超声治疗肝癌后免疫功能的变化 被引量:20

摘要 目的了解高强度超声聚焦热疗(HIFU)治疗后原发性肝癌患者的免疫功能变化情况。方法应用FEP BY01型肿瘤超声治疗机对42例原发性肝癌患者进行治疗。分别取HIFU治疗前和HIFU治疗后4周外周血10ml,检测T细胞亚群:CD4、CD8、CD4/CD8;Th细胞因子:IFNγ、IL2、IL4、Il10。结果42例肝癌患者治疗前和治疗后CD4、CD8T细胞百分数、CD4/CD8比值分别为(29.5±4.8)%和(38.2±5.1)%(P<0.05);30.2±6.4%和24.4±3.7%(P<0.05);0.9±0.3和1.7±0.2(P<0.01);治疗前和治疗后IL2、IFNγ、IL4、IL10含量(pg/ml)分别为93.5±28.3和110.5±24.2(P<0.05);73.2±18.6和92.8±20.1(P<0.05);82.3±14.2和60.2±11.3(P<0.01);74.8±21.9和56.2±19.1(P<0.01)。结论经HIFU治疗后肝癌患者的CD4T细胞百分数和CD4/CD8比值明显升高,CD8T细胞百分数下降;IL2和IFNγ水平明显升高,而IL4和IL10水平明显下降。应用HIFU技术治疗原发性肝癌可以改善患者体内的细胞免疫功能和Th1向Th2漂移的状态。 Objective To study the changes of immune function of patients with hepatocellular carcinoma (HCC) after high intensity focused ultrasound (HIFU) treatment. Methods Forty-two patients with HCC were treated by HIFU (type FFP-BY01). T-lymphocyte subsets CD4, CD8, CD4/CD8 and cytokines IFN-gamma, IL-2, IL-4, IL-10 in 10 ml of peripheral venous blood were examined before and four weeks after HIFU treatment. Results Before and after HIFU treatment, the percentages of CD4 T, CD8 T and the ratio of CD4/CD8 were((29.5±)4.8)% and (38.2±5.1)% (P<0.05), (30.2±6.4)% and (24.4±3.7)% (P<0.05), 0.9±0.3 and 1.7±0.2 (P<0.01), respectively. The serum levels (pg/ml) of IL-2, IFN-gamma, IL-4 and IL-10 before and after treatment were: 93.5±28.3 and 110.5±24.2 (P<0.05), 73.2±18.6 and 92.8±20.1 (P<0.05), 82.3±14.2 and 60.2±11.3 (P<0.01), 74.8±21.9 and 56.2±19.1 (P<0.01) respectively. The post-treatment levels of CD4, CD4/CD8, IFN-gamma and IL-2 were much higher than those before treatment, whereas the levels of CD8, IL-4 and IL-10 were significantly lower than those before treatment. Conclusions After HIFU treatment, the levels of CD4, CD4/CD8, IL-2 and IFN-gamma were much higher than those before treatment,while the levels of CD8, IL-4 and IL-10 were significantly lower than those before treatment.HIFU treatment of the patients with HCC promoted the recovery of T-lymphocyte subsets and Th1/Th2 cytokine levels from imbalance, and improved the immune function.
