MRCP对胰头癌的诊断价值

The Diagnostic Value of MRCP in the Diagnosis of Caput Pancreatic Cancer

目的 评价磁共振胰胆管造影 (magneticresonancecholangiopancreatography ,MRCP)对胰头癌的诊断价值。方法 对手术、病理证实为胰头癌的 2 8例病例的MRCP图像进行回顾性分析。结果 2 8患者全部成功地进行了MRCP检查 ,胰头癌在MRCP上的表现主要有胰胆管的阻塞、扩张及其它表现 ,某些病例可表现为特征性的“双管”征。MRCP对胰头癌的正确诊断阳性率 6 0 .7% ;MRCP +T2WI正确诊断阳性率 78.6 %。结论 MRCP是一种简单、安全、无创的影像检查技术 ,对胰头癌的诊断具有较高价值 ,其图像质量不低于ERCP。MRCP可在一定程度上替代ERCP ,但无法取代常规MR影像检查。

Objective To evaluate the diagnostic value of magnetic resonance cholangiopancreatography(MRCP) in the diagnosis of caput pancreatic cancer.Methods MRCP images data of 28 patients with caput pancreatic cancer diagnosed by surgery and pathology were analyzed retrospectively.Results 28 cases underwent MRCP examination successfully.The MRCP appearance of caput pancreatic cancer showed obstruction,dilatation and many other abnormalities,some caseseven appeared as typical “double duct”sign.The positive rate of diagnosing caput pancreatic cancer by MRCP was 60.7%;but that by MRCP plus T_2WI was 78.6%.Conclusion MRCP is a new simple,safe and noninvasive imaging technique,it has higher value in the diagnosis of caput pancreatic cancer.it can partially substitute MRCP for ERCP,but not for general MRI examination.
