Relationship Between TNF-α and IgA Nephropathy

摘要 目的 探讨TNF -α与IgA肾病的临床病理联系。方法 检测 5 0例IgA肾病患者血、尿TNF -α的浓度及肾组织TNF-α的表达 ,10例肾小球微小病变型患者及 10例健康人作为阴性对照 ,并选出缓解的患者作自身对照。结果 IgA肾病患者血、尿TNF -α水平均明显高于微小病变患者和正常人 ,与尿蛋白及病理的严重程度正相关 ;TNF -α阳性反应主要表达于肾近曲小管上皮细胞胞浆内 ,在单核巨噬细胞和淋巴细胞浸润较多的间质也有表达。结论 TNF -α参与了IgA肾病患者肉眼血尿、蛋白尿的发生 ,并与肾组织病变程度相关 ,是病情加重的一个重要因素。 Objective To investigate the clinicopathological relation between the TNF-α and IgA nephropathy. Methods TNF-α levels in serum, urine and renal tissues of 50 patients with IgA nephropathy were measured, and 10 patients with minimal change nephrotic syndrome(MCNS) and 10 healthy subjects served as negative control group. Results The serum and urine levels of TNF-α in the patients with IgA nephropathy were significantly higher than those in the patients with MCNS and healthy subjects, and had a significant positive correlation with the degree of proteinuria and renal demage. TNF-α expression was mainly localized in the cytoplasm of the proximal renal tubular epithelial cells, and the interstitium with more monocyte-macrophages infiltration. Conclusion TNF-α took part in the onset of hematuria and proteinuria, was correlated with the degree of renal damage, and played an important role in the aggravation of IgA nephropathy.
