Hoffmann征检查结果真假阳性对比研究的临床意义

Effect of wrist joint angle on the examination of Hoffmann reflex

目的 探讨腕关节不同屈伸角度对Hoffmann征检查结果的真假阳性干扰影响。方法 对 41 例明确诊断为脊髓型颈椎病的患者及50名正常人的双手在腕关节掌屈15°、中立0°、背伸30°、背伸60°位分别行Hoffmann征检查,并对所得结果进行统计学处理。结果 在脊髓型颈椎患者中,腕关节中立0°、背伸30°、背伸60°位Hoffmann征检查结果无显著差异, 而与掌屈15°位Hoffmann征检查结果存在显著差异(P<0.05)。在正常人中,腕关节掌屈15°、中立0°、背伸30°位 Hoffmann征检查结果无显著差异,而与背伸60°时Hoffmann征检查结果存在显著差异(P<0.05)。结论 腕关节不同屈伸角度对 Hoffmann征检查结果有明显影响,而以腕背伸30°位为可靠。

Objective To explore the effect of wrist angle on the examination of Hoffmann reflex.Methods Thirty-seven patients diagnosed as cervical myelopathy as well as 50 normal persons were examined for Hoffmann reflex respectively at 15°of palmar flexion, neutral position and 30°,60°of dorsiflexion.The results was statistically analyjed.Results In the cervical myelopathy patients, no significant difference was found among the results of Hoffmann reflex at neutral position, 30°,60°of dorsiflexion, but there was significant difference between the results of Hoffmann reflex at 15°of palmar flexion and that at neutral position,30°,60°of dorsiflexion. In the normal group, no significant difference was found among the results of Hoffmann reflex at 15°of palmar flexion, neutral position and 30°of dorsiflexion, but there was significant difference between the results of Hoffmann reflex at 60°of dorsiflexion and that at 15°of palmar flexion, neutral position and 30°of dorsiflexion.Conclusion Different wrist joint angles have significant effect on the result of Hoffmann reflex.The result at 30° of dorsiflexion is most reliable.
