肝肾囊肿硬化剂疗效评价及彩色多普勒超声导向作用

Evaluation of Absolute Alcohol Treatment for Hepatic and Renal Cyst by Percutaneous Puncture under the Guidance of CDFI

目的 观察超声引导下肝肾穿刺注射无水酒精治疗肝肾囊肿158例的疗效和彩色多普勒超声导向 作用。方法 在彩色多普勒超声引导下,经皮穿刺肝肾囊肿抽液后向肝肾囊肿内注射无水酒精。结果 158 例共173个肝肾囊肿经无水酒精治疗后复查总治愈率为81.5%、总显效率为97.7%。其中7例患者在治疗 后出现不同程度的头晕、恶心等,15例患者注入无水酒精后或拔出针后出现肝区、腰部胀痛或不同程度腹痛, 休息后均可缓解,无出血、感染、剧痛等严重并发征。结论 彩色多普勒超声导向能成功避开肝肾大血管防止 术中出血,并能进行囊肿的鉴别诊断。该方法具有疗效好、穿刺准确、方法简便、经济、安全等优点。

Objective To observe the therapeutic effect of absolute alcohol treatment on 158 cases with hepatic and renal cyst by percutaneous puncture under the guidance of color Doppler flow image (CDFI). Methods After transliver and transrenal aspiration of the encased fluid , injection of pure ethanol into the lumen was performed under the guidance of CDFI. Results After absolute alcohol treatment for 173 cysts in 158 cases, the cure rate was 81.5%, markedly effective rate was 97.7%. Among158 cases,seven cases had dizzi-ness and nausea with varying degrees after treatment. After injection of pure ethanol into lumen, or the needle extraction, fifteen cases had swelling pain in hepatic and lumbar region, or differential abdominal pain, which were remitted by taking rest. Conclusion Guidance of CDFI can keep the needle away from the hepatic and renal large vessels and be used in the differential diagnosis. This method is effective, safe, accurate, simple and economic.
