MRI不同扫描序列和MRA对早期脑梗死的诊断价值 被引量:3

Clinical evaluation of different MRI sequences and MRA for early cerebral infarction

摘要 目的:比较磁共振成像(MRI)FSE序列、FLAIR序列、磁共振扩散加权成像(DWI)和磁共振血管成像 (MRA)对早期脑梗死的诊断价值。方法:选择63例早期脑梗死患者,其中超急性期18例,急性期45例。全部患 者均行脑MRIFSE序列、FLAIR序列、DWI和3DTOFMRA检查。结果:18例超急性期脑梗死的DWI阳性检出率 为100%,FSE序列、FLAIR序列和MRA的阳性检出率分别为0%、27.8%和83.3%;45例急性期脑梗死的DWI、 FSE序列、FLAIR序列和MRA阳性检出率分别为100.0%、86.7%、91.1%和80.0%。结论:在早期尤其是超急性 期脑梗死诊断方面,DWI优于FSE序列、FLAIR序列和MRA;FLAIR序列优于FSE序列,对皮层和脑室旁病灶显示 更佳;MRI和MRA联合使用能够提供更丰富的诊断信息。 Aim: To compare the clinical value of FLAIR sequence, FSE sequence, diffusion weighted imaging (DWI) and 3D TOF MRA in diagnosing early cerebral infarction.Methods: A total of 63 cases of early cerebral infarction included 18 cases of superacute stage and 45cases of acute stage were collected. All cases were examined with FLAIR sequence, FSE sequence, DWI and 3D TOF MRA. Results: In 18 cases of superacute cerebral infarction, the positive diagnostic rates with DWI, FSE sequence, FLAIR sequence,and MRA are 100%, 0%, 27.8%,and 83.3%, respectively; the other 45 cases of acute cerebral infarction, the positive diagnostic rates with DWI, FSE sequence, FLAIR sequence,and MRA are 100.0%, 86.7%, 91.1%,and 80.0%, respectively.Conclusion: DWI is superior to FSE sequence, FLAIR sequence,and MRA in diagnosing early cerebral infarction, especially for superacute infarction. FLAIR sequence is superior to FSE sequence in diagnosing early cerebral infarction, especially for cortical and paraventricular lesions. MRI combined with MRA in detecting early cerebral infarction could provide with more information.
