摘要
应用美国ＡＴＬ公司Ｕｌｔｒａｍａｒｋ－８型超声诊断仪测定了２０名正常人和高血压病各期共５２例的房室腔内径、室间隔与左室后壁厚度。与对照组比较，Ⅰ期高血压病左房扩大，室间隔与左室后壁亦有增厚趋势；Ⅱ、Ⅲ期高血压病除上述变化加重外，右室内径增大，Ⅲ期高血压病左室腔显著扩大。作者认为，Ⅰ期高血压病左房扩大，可能是室间隔与左室壁增厚引起在室舒张功能下降的代偿性结果；Ⅱ期高血压病右室内径扩大则可能与室间隔及左心系统变化加重并通过心脏几何构型异常变化和肺循环阻力升高等机制有关；左室腔显著扩大和右室内径进一步增大，是Ⅲ期高血压病全心功能受损的结构基础。
In the present study, we measured the inside diameter of atrium and ventricle and the thickness of interventricular septum and left ventricular wall of 20 normal subjects and 52 patients with primary bypertension of different phases using Ultramark-8 type ultrasonic diagnosis instrument. The results showed that the inside diameter of left atrium, thickness of interventricular septum and left ventricular wall in patients with phase-I hypertension were significantly increased compared with those of controls(P<0.05),The inside diameter of right ventricle in patients with phase-Ⅱand Ⅲ hypertension was stastisyically increased and the cavity of left ventricle in patients with phase-Ⅲ hypertension was much larger than that of controls(P<0.05 or 0.001).These findings suggested that abnormal changes of atrium and ventricle may be structural basis of cardiac dysfunction in patients with primary hypertension.
