舒张充盈初期判断心脏舒张功能的临床应用

Clinical evaluation of cardiac diastolic function by CR interval

本文应用舒张充盈初期（ＣＲ间期）这一无创性指标，结合ＯＦ间期和总舒张时间（ＴＲＴ）分析，对６６例正常人和６４例高血压病人进行现察，发现正常高龄（＞４５岁）组的ＣＲ间期明显大于正常低龄（＜４５岁）组。高血压高龄（＞４５岁）组的ＣＲ间期显著低于正常高龄组。提示随着年龄增，左室舒张功能存在减退倾向。高血压高龄组除存在左室主动舒张功能减退这一因素外，尚有左房压的明显增高，认为ＣＲ间期结合ＯＦ间期和ＴＲＴ综合分析，是一个判断左室舒张功能和左房压的校好指标。

The diastolic function of the heart in 66 normal subjects and 64 hypertensive patients was studied by means of apexcardiographic protodiastolic filling period duration (CR interval)in combination with OF interval,and the total ralaxation time(TRT).The CR interval and the CR-OF interval calculated by subtracting the OF interval from CR interval,were signiFIcantly longer in the normal subjects over the age of 45 than in the younger normal subjects(129.7± 20.3ms,P< 0.01).Compared with the normal subcts over the age of 45,the hypertensive patients of the same age had a shorter CR interval and a longer TRT(119.4± 18.7 ms vs.129.7± 20.3 ms,P<0.05,and 112.5± 28.2 ms vs.95.5± 18.3 ms,P< 0.01,respectively).The results indicate that in the hypertensive patients over 45 years of age,the left ventricular diastolic function has a tendency of generation accompanied by aging,and there is a significant increase of left atrial pressure,in addition to the degeneration of the left ventricular relaxation. It is concluded that CR interval combined with OF interval and TRT is a useful method to assess left ventricular diastolic function and left atrial filling pressure.
