高脂血症血脂和凝血因子与脏腑及气血津液证型的关系探讨

Relation of Blood Lipids, Blood Coagulation Factors and Zang-fu, Qi-blood Type of Syndrome

目的 探讨高脂血症血脂和凝血因子的变化与中医辨证分型的关系。方法 将 3 0 2例高脂血症病人按脏腑及气血津液辨证分为 7个证型 ,检测病人血清TC、TG、LDL -C、HDL -C和血浆PT、APTT ,并将其检测结果在证型之间相互比较。结果 7个证型TC、TG、LDL -C和HDL -C均值变化是 3个实证 (肝阳上亢、痰浊中阻和气滞血瘀 ) >2个虚实夹杂证 (气虚血瘀和脾虚湿阻 ) >2个虚证 (气血亏虚和肝肾亏虚 ) ,TC和TG均值在高脂血症虚证属于轻度升高 ,在虚实夹杂证属于中度升高 ,在实证属于重度升高 ;PT在高脂血症各证型中没有差别 ,APTT仅在肝肾亏虚与痰浊中阻和气滞血瘀之间 ,在痰浊中阻与气血亏虚和肝肾亏虚之间有统计学意义 (P <0 .0 5 )。

Objective To explore the relation between the changes of blood lipid and blood coagulation factors in hyperlipidemia with Traditional Chinese Medicine (TCM) type. Methods 302 patients with hyperlipidemia were divided into 7 types of syndrome according to zang-fu and qi-blood and body fluid differentiation. In patients the serum TC, TG, HDL-C, LDL-C and plasma PT, APTT, were determined and compared between types of syndrome. Results In 7 types of syndrome the changes of mean value for TC, TG, LDL-C and HDL-C are 3 excess syndromes > 2 deficiency-excess miscellaneous syndromes >2 deficiency syndromes. The mean of TC and TG is slight elevation in deficiency syndromes, moderate elevation in deficiency-excess miscellaneous syndrome and serious elevation in excess syndrome. PT has no difference in various types of syndrome. APTT has remarkable difference between stagnation of phlegm-dampness and stagnation of qi and blood stasis with deficiency of liver and kidney (P<0.05). and has also obvious difference between deficiency of qi and blood and deficiency of liver and kidney with stagnation of phlegm-dampness (P<0.05). Conclusion The blood lipids indexes of this study are related with various types of syndrome of hyperlipidemia in the different degree.
