改良耻骨上经膀胱前列腺切除术(附350例报告)

Modification of suprapubic prostatectomy(report of 350 cases)

目的 探讨改良耻骨上经膀胱前列腺切除术的治疗方法及效果。方法 采用窝口周围环扎血管、楔形切除膀胱颈后唇、尿管气囊隔离膀胱与腺窝、膀胱造瘘口预置缝线及术后连续硬膜外腔自控镇痛等。结果 采用以上措施治疗前列腺增生患者350例,取得了明显疗效,经5个月至8年随访,没有严重并发症发生。结论 本术式手术时间短、出血少,大部分患者不用输血、安全性高、并发症少,适合基层医院开展。

Objective To explore the curative method and effects of modification of suprapubic prostatectomy.Methods To circular suture of the prostatic vessels during operation,wedge resection bladder cervix rear lip,separate the urethral orifice from bladder by Foley's air sac,set aside the thread through manmade bladder orifice and let epidural post-operation analgesia continue,etc.Results By the above methods 350 cases have achieved good effects.There has not been a serious complication for 5-months to 8-years followed up.Conclusion This type of operation has shortened surgical time,less bleeding,which has higher safety and less complications,fit for basic level hospital.And most patients do not need to have a blood transfusion.
