速尿和氨茶碱对小猪再缺氧损伤的预防效果 被引量:5

The preventive effects of furosemide and aminophylline on the rehypoxic injury in small pigs

摘要 目的 寻找预防缺氧动物吸入纯氧后再损伤的药物。 方法  15只自平原空运至高原 (36 5 8m)的小猪被分为 3组 :对照组、速尿组和氨茶碱组。采用右心漂浮导管检测法 ,观察猪吸入纯氧后恢复吸空气前静推氨茶碱或速尿对再缺氧损伤的预防效果。 结果 对照组动物肺动脉平均压在吸入 10 0 %氧气后即明显下降 ,在突然停止吸氧而改吸室内空气后 ,下降的肺动脉压逐渐回升 ,5min后恢复到吸氧前水平。随着时间的延长 ,肺动脉压继续攀升 ,15min时肺动脉压达最高点 ,明显高于吸氧前水平。与此同时 ,动脉血氧分压较吸氧前低。而当猪在停止吸氧的同时 ,静推氨茶碱或速尿后 ,下降的肺动脉平均压虽有所回升 ,但回升速度明显低于对照组 ,在停止吸氧 30min时 ,仍低于吸氧前水平。 结论 猪存在再缺氧损伤 ,速尿、氨茶碱均可预防缺氧动物再缺氧损伤。 Objective To investigate drugs for prophylaxis of rehypoxic injury. Methods Fifteensmall pigs newly (1 wk before) transported from plain to plateau (3658 m) served as subjects. They were randomly divided into 3 groups: control group, furosemide group, aminophylline group. Mean pulmonary arterial pressure (mPAP) and PaO 2 were measured before 100% O 2 inhalation, after 100% O 2 inhalation for 30 min and every 5 min after discontinuing O 2 inhalation for 30 min. For the later two groups, furosemide (0.4 mg/kg) or aminophylline (5 mg/kg) was given intravenously at the time of discontinuing 100% oxygen supply. Results In pigs of control group, mPAP was lowered significantly (P<0.01) after 30 min O 2 inhalation as compared with that before O 2 inhalation, after changing to room air inhalation mPAP became higher and higher and reached the peak value 15 min afterO 2 discontinuation which was significantly higher (P<0.01) than initial value. On the other hand, arterial PO 2 15 min after discontinuation of O 2 inhalation reached the lowest point which was significantly lower than the initial value. As to pigs of 2 drug groups, the rebounding of mPAP after O 2 discontinuation was alleviated and did not exceed the initial value in 30 min after O 2 discontinuation, and PO 2 never lower than the initial value in the course of observation. Conclusions The pigs newly transported to plateau also revealed characteristics of rehypoxic injury after discontinuation of 100% O 2 inhalation. Both furosemide and aminophylline have preventive effect on rehypoxic injury.
