水突穴注射合用星状神经节阻滞对脑卒中后吞咽障碍的影响

Effect of Shuitu acupoint injection with stellate ganglion block on swallow dysfunction after stroke

目的:探讨水突穴注射合用星状神经节阻滞疗法对脑卒中后吞咽障碍的改善作用及机制。方法:1999-05/2004-01韶关市中医院收治脑卒中后患者63例,随机分为治疗组32例,对照组31例,两组常规治疗相同,治疗组加用水突穴注射合用星状神经节阻滞治疗,比较两组治疗后脑卒中后吞咽障碍的改善。结果:经2个疗程(40d)治疗后,治疗组Fugl-Meyer运动功能评分、改良的Barthel指数由9±21,35±15升至65±22,85±20,神经功能缺损由27±9降至8±5,对照组相应评分改善均不如治疗组,差异有显著性意义(P均<0.05);吞咽能力评分:治疗组由3.50±0.73升到5.51±0.81,对照组由3.67±0.34升到5.01±0.62,治疗后两组对比差异有显著性意义(t=5.83,P<0.05)。结论:水突穴注射合用星状神经节阻滞疗法对脑卒中后吞咽障碍有明显改善作用,且安全。

AIM: To investigate the effect of Shuitu acupoint injection with stellate ganglion block(SGB) on swallow dysfunction after stroke and the mechanism.METHODS: Totally 63 stroke patients from Shaoguan Hospital of Traditional Chinese Medicine between May 1995 and January 2004 were randomly divided into treatment group(n=32) and control group(n=31).Routine therapy was administrated in the two groups. Furthermore, the patients of the treatment group were treated by Shuitu acupoint injection with SGB. The amelioration of swallow function after treatment between the two groups was compared.RESULTS: At 40 days after treatment, the motor function scores of Fugl- Meyer assement and the Barthel indexes in the treatment group increased from 9± 21 and 35± 15 to 65± 22 and 85± 20; however, the scores of neurologic impairment in the treatment group declined from 27± 9 to 8± 5, the amelioration of the scores in control group was not as good as that in treatment group,and there was significant difference(P< 0.05).The scores of swallow function increased from 3.50± 0.73 to 5.51± 0.81 in treatment group, and increased from 3.67± 0.34 to 5.01± 0.62 in control group, and there was significant difference(t=5.83,P< 0.05).CONCLUSION: Shuitu acupoint injection with SGB can significantly ameliorate the swallow dysfunction in patients with stoke.
