房颤电转复后左房收缩功能的超声心动图评价 被引量:2

摘要 目的 :应用超声心动图评价房颤患者电转复后的左房收缩功能。材料和方法 :对 2 0例正常人和 40例成功电除颤患者除颤后 1、3、7天和 1个月分别测量二尖瓣血流频谱A峰流速 (VA)、A峰速度时间积分 (A -VTI)、左房射血力(AEF) ,利用定量组织速度成像 (QTVI)测量二尖瓣环舒张晚期的运动速度 (Va) ,并将Va与前三项指标做相关性分析。结果 :房颤患者除颤后 2 4小时VA、A -VTI、AEF和Va明显低于正常 ,随时间推移逐渐增高 ,至除颤后 7天时基本恢复至正常水平。Va与前三项指标均呈显著正相关 ,r分别为 0 .87、0 .84、0 .76。结论 :房颤患者电除颤后心房功能出现顿抑 ;QTVI测量二尖瓣环心房收缩期心肌运动速度可用于评价左房收缩功能。 Purpose: To evaluate atrial function after electrical cardioversion of atrial fibrillation by echocardiography. Materials and Methods: A total of 40 patients with atrial fibrillation underwent echocardiographic examination 24 hours after the restoration of sinus rhythm by direct-current cardioversion. The echocardiography was repeated at 3, 7 days and 1 month. Another 20 normal individuals were also included in the study as controls. Results: The peak A velocity(VA), velocity time integral of A-wave(A-VTI), atrial ejection force(AEF) and peak atrial tissue velocity(Va) decreased in day 1 after cardioversion, but gradually recovered with time. Va correlated well with VA, A-VTI and AEF(r=0.87,r=0.84,r=0.76). Conclusion: Atrial stunning was observed after electrical cardioversion. And peak atrial systolic mitral annular velocity assessed by echocardiography was of value in revealing left atrial systolic function.
