368例声带麻痹病因分析

Retrospective Study on Etiology of 368 Patients with Vocal Fold Paralysis

目的 分析、了解声带麻痹病因的分布情况 ,为诊治提供参考依据。方法 对 1993～ 2 0 0 3年在我科内窥镜室经硬管喉镜和纤维喉镜检查确诊的 36 8名声带麻痹患者的临床资料进行回顾性分析。结果 其病因可分为 7大类。在这 7类病因中 ,由肿瘤及占位病变引起的居首位 ,共有 133例 ,占 36 .1% ,左右两侧无显著性差异 ;手术损伤居第 2位 ,共 99例 ,占 2 6 .9% ;其它类声带麻痹位于第 3位 ,共 88例 ,占 2 3.9% ;“感冒”发烧后引起者居第4位 ,有 33例 ,占 9% ;创伤居第 5位 ,有 7例 ,占 1.9% ;脑血管意外引起声带麻痹 5例 ,占 1.4 % ;排在最后位的是结核引起的声带麻痹 ,共 3例 ,占 0 .8%。结论 头颈部肿瘤引起的声带麻痹最多见 ,胸部病变及胸部手术引起的也较多 ,在诊断和治疗上须引起注意。病毒感染也是引起声带麻痹的原因之一。

Objective In order to know the distri bu tion of etiology of vocal fold paralysis,we retrospectively investigated causes of patients in recent 10-years. Methods From December 1993 to December 2003, 368 cases we re diagnosed as vocal fold paralysis in our endoscope room. Results 133 cases resulted from tumors and occupied disea ses,99 cases from operative injury, 88 cases from idiopathic, 33 ca ses from virus infection, 7 cases from trauma, 5 cases from cerebrovascular dise ases, 3 cases from tuberculosis. Conclusion Tumors in head and neek are the most commonly seen causes of volal fold paralysis,followed by pulmonary and chest surgery.Viru s infection is also one of causes of it.Therefore,We must pay attention to ches t diseases if we find vocal fold paralyses.
