数字减影血管造影和栓塞疗法在鼻咽纤维血管瘤手术前的应用价值

Value of Digital Subtraction Angiography and Embolization before the Operation of Nasopharyngeal Angiofibroma

目的探讨数字减影血管造影(digital subtraction angiography,DSA)和选择性血管栓塞在鼻咽纤维血管瘤手术前的应用价值。方法回顾分析我院1990～2002年收治的6例经手术和病理证实的鼻咽纤维血管瘤中4例按 Seldinger’s技术进行了超选择性动脉造影并栓塞治疗。结果 4例均显示病变范围血供情况以及营养血管的特征,栓塞后术中出血减少到300 ml 以下。结论数字减影血管造影和选择性血管栓塞能在切除肿瘤前进一步证实诊断并且显示肿瘤范围,术前栓塞能大大减少术中出血。

Purpose To investigate the value of digital subtraction angiography(DSA)and selective vessel embolization before the operation of nasopharyngeal angiofibroma.Methods 6 cases of nasopharyngeal angiofibroma proved by operation and pathology from 1990 to 2002 were analyzed retrospectively,in which 4 cases were treated with DSA and vessel embolization according to Seldinger's technology.Results All of the 4 cases were demonstrated by blood supply of lesions and the feature of nutrient vessels.After embolization,the amount of bleeding was reduced below to 300 ml in operation.Conclusion DSA and selective vessel embolization could prove the disgnosis and demonstrate the range of tumor before tumors being removed.Embolization before the operation could significantly reduce the bleeding in operation.
