骨科老年糖尿病患者围手术期血糖控制标准的临床研究

The Control Criterions of Blood Glucoses for Aged Orthopedic Patients with Diabetes in Perioperative Period

目的探讨骨科老年患者并存糖尿病的情况下依手术大小的不同在围手术期时对血糖控制的不同标准。方法根据骨科手术复杂程度,创伤的大小,手术时间的长短,人工内植物构造的复杂程度等因素,将手术分为Ⅰ、Ⅱ、Ⅲ三类,然后依据手术的分类,将糖尿病患者术前血糖控制的标准,也分为三个等级,据此指导临床,回顾206例60岁以上的患者,按照这个标准进行血糖调控和施行手术的情况。结果206例老年糖尿病患者接受骨科手术,术后感染率为0.49%,取得了较好的治疗效果。结论控制好围手术期血糖水平,根据手术大小,将血糖分别控制在相应的许可范围内,老年糖尿病患者接受骨科手术是安全的。

Objective To investigate the blood glucose control criterions for aged orthopedic patients with diabetes in perioperative period.Methods The operations for the aged patients with diabetes were divided into group Ⅰ,group Ⅱ and group Ⅲ basing on the trauma degree,the duration of operation and the complexity of prosthesis.The control criterions of blood glucose were accordingly divided into three grades.Two hundred and six orthopedic operations were reviewed.Results The postoperative infection rate was only 0.49%.All operations achieved excellent therapeutic effect.Conclusion The orthopedic operation for aged patients with diabetes is safe on the condition that the blood glucose is controlled within permissible ranges.
