实验性急性多发微小血栓栓塞症IL-6、TNFa的变化及其意义

摘要 目的 :探讨家兔多发微小血栓导致急性肺栓塞后某些细胞因子水平变化及其意义 ,了解它们对肺血栓栓塞症 (Pulmonarythromboembolism ,PTE)病理生理过程的影响。方法 :2 6只健康中国大耳白兔随机分为正常对照组、肺栓塞组 ,每组 13只。肺栓塞组家兔经股静脉注入自体血凝块制备PTE模型 ,且动态观察PTE前后IL 6、TNFa的变化。结果 :肺脏栓塞部位肉眼观 ,见肺表面点片状出血灶 ;镜下肺小动脉血管内可见多发微小血栓形成 ,肺泡及血管周围有大量炎性细胞浸润 ,肺泡和间质内有局灶出血。肺栓塞后血中IL -6、TNFa水平持续升高。结论 :PTE后TNFa、IL- 6参与炎症反应及新鲜血栓形成 ,从而影响PTE的病理生理过程。 Objective: To study the changes and significance of some cytokines before and after experimental acute multiple pulmonary microthromboembolism in rabbits.Methods: Rabbit pulmonary microemoblism models by injection of autologous blood clots into femoral vein were used to observe the dynamical changes of tumor necrosis factor_a and interleukin_6 in blood.Results: Petechial and patchy hemorrhages were observed on the surfaces of embolic lungs.The injected blood clots and secondary multiple microembolism in the pulmonary small arteries,inflammatory cell infilrtration around alveoli and emboloic arteries,local hemorrhages in the alveoli and interstitial tissue could be found by microscopy.The concentration of TNFaIL_6 in blood were significantly elevated after microembolism.Conclusion: TNFaIL_6 potentially involved in the inflammatory response and secondary thrombosis after pulmonary microembolism and contributed to the pathophysiological alterations in connection with pulmonary microembolism
