腰椎间盘退变时椎间隙形态与腰椎曲度变化及其临床意义 被引量:23

摘要 目的:研究腰椎间盘退变时椎间隙的形态变化以及腰椎曲度的变化,并探讨其临床意义。方法:121例研究对象分成3组,对照组33例,L4/5椎间盘轻度退变组53例,重度退变组35例。将所有研究对象的MRI片输入电脑,在MRI正中失状面的T2WI图像上观察椎间盘的信号,椎间隙的前高、中高和后高,椎间隙的角度以及腰椎曲度。结果:(1) 椎间隙高度在对照组、腰椎间盘轻度和重度退变组逐渐减小,两两间有显著性差异(P <0.05); (2)椎间隙角度以及腰椎曲度在对照组、腰椎间盘轻度和重度退变组逐渐减小,两两间有显著性差异(P <0.05); (3)椎间隙的角度和腰椎曲度呈正相关, (P<0.05)。结论:腰椎间盘退变时椎间隙的形态以及腰椎曲度发生变化,这些变化对腰椎间盘退变的治疗有指导意义。 Objective: To examine the influence of lumber disk degeneration on vertebral disc shape and lumbar lordosis. Methods: 121 cases were adopted and divided into three groups: control group of 33 asymptomatic volunteers, slight degeneration group of 53 cases and severe degeneration group of 35 cases. For all cases, sagittal magnetic resonance imaging of lumbar spine were inputted into personal computer. The anterior, middle and posterior heights of L4/5 intervertebral space, as well as the signal, the angle of the L4/5 disc and lumbar lordosis were measured. Results: The height of intervertebral space and the angle of L4/5 disc and lumbar lordosis gradually decreased among the control group, slight degeneration group and severe degeneration group (P <0.05). The angle of L4/5 disc had positive correlation with lumbar lordosis (P <0.05). Conclusions: For patients with lumbar disc degeneration, changes of the disc shape and lumbar lordosis present. To investigate those changes is very valuable for the therapy of lumbar disc degeneration.
