防止乳腺导管内乳头状瘤癌变的探讨

The discussion of preventing the canceration of Breast Intraductal Papillomas

目的 :采用细胞学方法诊断导管内乳头状瘤 (intraductal,papillomas,IP)癌变 ,并探讨其意义。方法 :714例乳头溢液病例全部行脱落细胞学检查。大部分病例同时做近红外线扫描、 B超、乳腺 X线检查。结果 :查出 IP 16 1例 ,其中发现癌细胞或可疑癌细胞 2 0例 ,占 12 .4 % ,与术中术后病理相符 ,癌变是在 IP基础上发生了癌细胞样改变。癌变平均年龄 4 7.2岁 ,血性溢液为主 ,占 75 .0 % ,结论 :乳头状瘤是乳腺癌的前期病变。故女性年龄超过 4 5岁 。

Objective:To use cytological methods in the diagnosis of cancerous changes of intraductal papillomas (IP) and discuss its significance Methods: 714 cases of nipple discharge smear were studied by using Wright-Gimsa staining Meanwhile, infrared ray scanning,B-ultrasound and X-rays were performed on most of these cases Results: 161 patients were diagnosed with IP Among them, 20 patients or 12 4% were found with tumors or were suspected to have tumors This was confirmed by examining the pathology of patients during surgery or post surgery The cancerous change of cytomorphology was based on IP The average age of patients with cancerous change was 47 2 The nipple discharge in patients with cancerous change was primarily bloody discharge (75% or 15 out of 20) Conclusion: IP becomes earlier state of breast tube cancer after cancerous change Women who are older than 45 years old, with nipple discharge and in post menopause are considered to have higher possibility of cancerous change
