儿童骨原发性非霍奇金淋巴瘤3例临床病理分析及文献复习

Primary non-Hodgkin's lymphoma of bone in children:clinicopathological analysis of 3 cases and review of literature

目的 探讨儿童骨原发性淋巴瘤的临床病理学特征 ,评估免疫组化染色在鉴别诊断中的意义。方法 对3例儿童骨的恶性淋巴瘤 ,复习临床资料并进行病理学检查及免疫组化染色分析。结果 3例均诊断为骨原发性非霍奇金淋巴瘤 ,B细胞来源 ,其中 1例为B淋巴母细胞型 ,1例为弥漫大B细胞型 ,1例不能确定类型。 3例均行病变部位的肿瘤切除术 ,术后化疗。结论 儿童骨原发性淋巴瘤是一种罕见的恶性肿瘤 ,临床及放射学上难与其他儿童骨恶性肿瘤鉴别 ,免疫组化染色在诊断中不可或缺。

Objective To investigate the clincopathological features of primary non-Hodgkin's lymphoma of bone in children and to evaluate the significance of immunohistochemistry in its differential diagnosis. Methods In this study, two girls and one boy with primary lymphoma in bone were reviewed. Tumors biopsy and immunohistological stain were preformed. Results All patient had primary non-Hodgkin's lymphoma of bone based on the World Health Organization( WHO) classification of tumours. The histologic subtypes included one case of B lymphoblastic lymphoma, one diffuse large B cell lymphoma and one unclassificable lymphoma. The excisions of tumours were performed in all cases. The patients were treated with CHOP chemotherapy with or without radiation. Conclusion Primary non-lymphoma of bone in children is a rare malignant tumor. The differential diagnosis from other malignant tumors in children is difficult in clinical and radiographic presentation. The immunohistochemical staining is necessary for correct diagnosis and approprate chemotherapies are important for patients' prognosis.
