摘要
目的探讨磁共振扫描黑白血技术评价左心室形态及功能。方法应用1.5TCV/i(GE milvaukee)磁共振仪及工作站对40例健康成年人进行MRI扫描及后处理分析。利用黑血技术双-反转恢复快速自旋回波序列和三-反转恢复快速自旋回波序列,分别行左心室短轴位、长轴位、四腔位扫描并测量左心室室壁和室间隔的厚度、心腔大小。白血技术应用Fast Cine扫描短轴位并测量左心室功能,得到健康成年人左心室的形态和功能的正常值。结果磁共振短轴位黑血技术中双-反转恢复快速自旋回波序列提供左心室从心尖至心底等不同部位的室间隔、心室游离壁(包括前壁、侧壁、后壁)及前、后组乳头肌的厚度、心腔形态、大小等信息,长轴位黑血技术主要提供左心室心尖的厚度、心腔大小等信息,四腔位黑血技术提供心尖和室间隔的厚度、心腔大小等信息。三-反转恢复快速自旋回波序列提供脂肪抑制后左心室壁信号的变化。白血技术能反映心肌的运动,计算射血分数、心肌重量、心肌收缩增厚率以及瓣膜的功能。结论黑白血技术磁共振检查能提供心脏各平面优良的图像并能准确测量左心室的室壁厚度、心腔的大小,且能计算心室功能。短轴位观察除心尖外的室间隔、心室游离壁及乳头肌的厚度最佳。长轴位观察左心室心尖最好。四腔位观察左心室心尖最好。
Objective To probe role of black and white angiogram in evaluating anatomy and function of left ventricle. Methods A GE signa 1. 5 Tesla CV/ i MR system was used to make scanning and process data from 40 healthy adults.Black angiogram provided with double IR FSE and triple IR FSE sequences scanning along short and long axis of left ventricle and four chambers,and measuring thickness of ventricular wall and interventricular septum and size of heart cavity.White angiogram provided with Fast Cine scanning along short axis and measuring left ventricle. Results IVS and LV free wall thickness, cavity length were measured with black-blood techniques double IR FSE from the apical level to the basal level in short axis view.LV apical wall thickness and cavity length were measured with black-blood techniques double IR FSE in long axis view.IVS and LV apical wall thickness, cavity length were measured with black-blood techniques double IR FSE in four chamber view.LV signal changes were demonstrated with black-blood techniques triple IR FSE.Myocardium actions were demonstrated with white-blood techniques fast cine.LV function including EDV,EDS, mass,EF and valve function were calculated with short axis view. Conclusion Black and white angiogram was superior in measuring IVS ,LV wall thickness, cavity length and LV function.Short axis view was best for evaluating LV IVS and LV free wall thickness,and long axis view was best for evaluating LV apical wall thickness and LV wall thickness,and four chamber view was best for LV apical wall thickness,IVS,LV side wall thickness, and cavity length, approaching acknowledged normal values. It is concluded that black and white angiogram is able to help images from various views to complement each other, offer fuller anatomic structure and functions of heart, and deserves recommen- dation for clinical use.
