交腿腓肠肌肌皮瓣的临床应用及并发症的预防

Clinical application and complication of gastrocnemius flap

[目的 ]探讨交腿腓肠肌肌皮瓣修复小腿皮肤软组织缺损的方法。 [方法 ]采用交腿腓肠肌肌皮瓣修复小腿皮肤软组织缺损 14例。在治疗中采用取出内固定钢板改用外固定架固定 ,预留肌皮瓣观察窗的方法。 [结果 ]14例患者在治疗后治愈 13例 ,1例患者于断蒂术后发生肌皮瓣远端坏死 ,经过治疗后愈合。 [结论 ]交腿腓肠肌肌皮瓣是修复小腿皮肤软组织缺损的较好方法。在治疗中取出内固定钢板改用外固定架固定 ,预留肌皮瓣观察窗的方法能有效减少并发症的发生。

Objective To explore the method of repairing the missing of the soft tissue of the leg. [Methods] Soft tissue defects on the leg in 14 patients were repaired with gastrocnemius flap and performed the way of taking the plate out and fixed it with the external fixators instead. And then kept the observation window of the flap during the treatment. [Results] The flaps completely survived in 13 cases. Distal flap was partly necrotic in 1 case. [Conclusion] The gastrocnemius flap is the comparatively better method of restoring the soft tissue detects of the leg, the way of taking the plate out and fixing it with the external fixators instead, and then keep the observation window of the flap during the treatment can reduce the incidence of the complication.
