巨大矢状窦、镰旁脑膜瘤的外科治疗

The Surgical Management of Giant Parasagittal and Falcial Meningiomas

[目的]提高巨大矢状窦、镰旁脑膜瘤的外科治疗效果。[方法]回顾性分析44例巨大矢状窦、镰旁脑膜瘤临床资料。[结果]44例病例中,SimpsonⅠ级切除者20例(45.5%),SimpsonⅡ级切除者22例(50.0%),SimpsonⅢ级切除2例(4.5%),手术死亡1例。随访2￣102个月,2例Ⅲ级切除者复发。[结论]充分的术前影像学评估、有效地控制术中出血、处理好上矢状窦和避免脑皮质损伤是提高矢状窦、镰旁脑膜瘤手术疗效的重要因素。

To investigate surgical efficacy of patients with giant parasagittal and falcial meningiomas.The clinical data of 44 patients with parasagittal and falcial meningiomas were analyzed retrospectively. Of 44 patients, 20 (40.5%) patients underwent Simpson gradeⅠ resection, Simpson gradeⅡ resection in 22 cases (50.0%)and Simpson grade Ⅲ resection in 2 cases( 4.5%). One patient occurred operation death .All patients followed up with a range from 2 to 102 months,2 patients with Simpson grade Ⅲ resection recurred.[Conclusion]Sufficient preoperative imaging evaluation ,efficient control of intraoperative blood loss, excellent management of superior sagittal sinus(SSS) and falx and avoidence of damage to vital brain area will facilitate to improve the surgical results in patiens with parasagittal and falcial meningiomas.
