低分子肝素治疗心肌梗塞并发出血的观察及护理

我院于1999年6月～2003年12月应用低分子肝素(LMWH)抗凝治疗急性心肌梗塞126例,收到较好的临床效果,治愈好转109例,占86.5%,末愈及死亡17例,占13.5%例,其中有76.1%患者有轻度皮下出血(淤斑)、末经处理后自愈,4.6%有轻度尿道及胃出血,无1例大出血。通过严格掌握注射方法,尤其是注射完后局部压迫10～20分钟,掌握低分子肝素抗凝注射的适应症和禁忌症,严密观察病情、注意有无出血的倾向,预防中度和重度出血并发症的发生。做好患者的心理护理以取得患者的积极配合,低分子肝素抗凝治疗急性心肌梗塞是安全、可靠的,方法简便、效果好。

My hospital had cured 126 AMI patients in LMWH from June 1999 to December 2003. The effect was preferable,and,109 cases(86%) were cured ,17cases (13.5%) were un-cured or death. There 76.1% cases occurred little petechia and self-cured,4.6% cades were light hematuria and gastric hemorrhage, no case was massive haemorrhage. Therefore, the method of using LMWH in AMI is safe, tried, convenient and good effect with strict holding the injecting local and especially pressing the injecting part for 10 to 20 minutes, commanding the applicability and prohibit, strictly observing the trend of hemorrhage ,preventing the haemorrhate ,making mental nurse to get patient’s cooperation.
