急性重症哮喘的诊治体会(附52例报告)

The Diagnostic and Therapeutic Comprehesion for Acute Severe Asthma

目的:总结重症哮喘的诊治经验。方法:通过对 52 例重症哮喘病人的住院资料作回顾性分析总结。结果:48例治愈,治愈率:92.3%(48/52);4 例死亡,占同期住院哮喘患者的 3.92%(4/102);其中13例进行了机械通气治疗,9例在3d内脱机,2例并发纵隔气肿2周后症状改善脱机拨管,2例死于多器官功能衰竭;另2例拒行机械通气者 3d内死于呼吸衰竭。结论:及时准确评估病情,合理应用皮质激素等药物及机械通气支持等综合治疗措施是提高抢救成功率的关键。

Objective: To conclude the diagnostic and therapeutic comprehesion and experience on severe asthma. Method: Retrospective analysis and conclusion was drawn from the data of 52 hospitalized patients with severe asthma. Result: 48 cases were healed.The healing ratio was 92.3%(48/52);4 cases died and which take the proportion of the hospitalized patients with asthma at the same period was 3.92%(4/102). Among which 13 cases were mechanical ventilated,9 cases were weaned in 3 days,2 cases complicated with the pneumomediastinal were weaned after 2 weeks,2 cases died of multiple complicated with the pneumomediastinal were weaned after 2 weeks,2 cases died of multiple organ function failure,another 2 cases who refused to be ventilated were died of respiratory failure. Conclusion: The key point to raise the rescue rate was the therapeutic measures including correctly evaluating the patient’s condition in time,reasonally utilizing the corticosteroid and mechanical ventilatory support.
