海洛因中毒性脑病临床分析

Clinical Analysis of Toxic Encephalopathy Induced by Inhaling Heroin

目的:探讨海洛因中毒性脑病的临床特点及影像学改变。方法:对 21 例海洛因中毒性脑病患者的临床及影像学资料进行观察和分析。结果:21例患者临床特点主要为亚急性起病,小脑性共济失调和精神症状为突出表现的脑弥漫性损害。CT或MR主要表现累及双侧颞、枕叶白质区及小脑半球齿状核和脑干,具有散在、多发、对称性和无占位效应为特点。2 例死亡原因主要是肺水肿、呼吸衰竭。结论:本病诊断主要依靠明确海洛因吸毒史及具有特征性的临床表现和影像学特征。

Objective: To discuss clinical manifestation and imagine features on toxic encephalopathy after inhaling heroin. Method: 21 patients with toxic encephalopathy induced by inhaling heroin were observed and analyzed on their clinical and imagine features. Result: The main clinical manifestations were of the sub-acute diffuse cerebral lesions, especially of early mental symptoms and cerebellum ataxia. The CT or MRI of patients showed bilateral white matter lesions involving in the parietal, occipital, temporal, cerebellum hemisphere dentate nucleus and brain stem. Their features were diffuse, multiple, symmetric and not occupational mass shown in general. 2 cases died in pulmonary edema and respiratory failure. Conclusion: The diagnosis of HTE should be dependent on the definite inhaling heroin and their clinical features and radiological appearance.
