射频消融术治疗子宫肌瘤的观察及护理

Observation and nursing care of myomas of uterus treated with radiofrequency ablation

目的探讨射频消融术治疗16例子宫肌瘤的临床治疗效果及护理。方法选择直径小于5cm的子宫肌瘤患者16例,在B超定位下,用射频治疗仪专用穿刺针(以下称自凝刀)治疗,经充分的术前准备,术中严密监护患者神志、面色、生命体征,术后观察腹痛、阴道流血、发热情况,防治感染等。结果16例患者全部手术成功。手术后3个月、6个月复查B超,14例子宫肌瘤消失,2例子宫肌瘤明显缩小。1例术中出现心率减慢至46次/min,立即予阿托品0.5mg静脉注射后心率恢复至76次/min。全部患者均无腹腔脏器损伤和盆腔感染等并发症发生。结论射频消融术治疗子宫肌瘤具有不用开刀、安全、有效、操作简单、痛苦小、反应轻、并发症发生率低等优点,是一种安全可靠、疗效肯定的治疗子宫肌瘤的微创新方法。

Objective To explore the therapeutic effect and nursing of 16 myomas of uterus treated with radiofrequency ablation. Methods 16 patients with uteri ne myoma were treated with radiofrequency ablation under-ultrasound. Before oper ation, full preparation was performed, the sense, complexions and vital signs we re observed during treatment, and after treatment, pains in belly, bleeding of v agina, fever were closely monitored to prevent infection. Results All the patie nts were successfully treated. Re-examinations 3 months and 6 months after opera tion were conducted by B-ultrasound: 14 myomas of uterus disappered, 2 shrank. D uring treatment, arrhythmia once slowed down to 46 beats/min and went up to 76 b eats/min after infusing 0.5 mg atropine. There were no injuries in organs in abd ominal cavities and no infections in pelvic cavities. Conclusion The treatment of myomas of uterus with radiofrequency ablation has the advantages of using no operating scalpel, safety, effectiveness, simplicity in operation, less pains, less adverse reaction, lower rate of complications. As a new minimally invasive therapy for myomas of uterus, it is safe and effective.
