高血压并发睡眠呼吸暂停综合征病人人工呼吸机辅助治疗的临床研究

Influence of CPAP on Blood Pressure in Patients with Hypertension and Sleep apnea Syndrome.

目的 研究高血压合并睡眠呼吸暂停综合征 (SAS)病人呼吸道正压通气治疗与常规药物治疗对血压的影响。方法 根据多导睡眠呼吸监测和2 4h血压监测资料 ,入选 43例高血压合并SAS病人 ,随机分为常规组 (2 2例 )和呼吸道正压通气治疗组 (2 1例 ) ,均给予常规降压药物治疗 4周 ,在常规治疗基础上加用呼吸道正压通气治疗一夜 ,观察两组治疗前后 2 4h血压变化。结果 两组病人治疗后2 4h平均收缩压、舒张压、夜间舒张压均明显下降 (P <0 .0 5 ) ,但仍高于正常值 ,而夜间收缩压无明显变化。呼吸道正压通气治疗组治疗后 2 4h平均收缩压、舒张压进一步降低 ,夜间收缩压和夜间舒张压显著降低。结论 对高血压合并睡眠呼吸暂停综合征病人 ,在常规降压药物治疗基础上需应用呼吸道正压通气治疗。

Objective To explore the influnce of CPAP on blood pressure in patients with hypertension and sleep apnea syndrome. Methods According to the data of 24-hour ABPM and polysomnography(PSG) ,patients with hypertension and sleep apnea syndrome were randomly divided into two groups. All patients had accepted antihypertensed therapy for 4 weeks. One group accept CPAP treatment at the base of routine therapy.We compared the data from 24-hour ambulatory blood pressure before and after therapy. Results 24-hour BP and nighttime BP were all decreased(P<0.05)after 4 weeks’ antihypertensed therapy in two groups.But still higher than normal CPAP treatment.In the group with 24-hour BP and nighttime BP was significantly decreased(P<0.01). Conclusion To most of patients with Hypertension and sleep apnea syndrome,CPAP treatment can effectively decrease 24-hour BP and nighttime BP.
