原发性高血压患者脉压与左室功能的相关性研究 被引量:4

摘要 目的原发性高血压患者脉压(PP)与左室收缩和舒张功能的相关性研究。方法92例原发性高血压患者和53例健康对照者为研究对象。标准台式水银柱血压计测右上肢肱动脉血压;彩色多普勒超声心动图检查仪测量左室重量及左室收缩、舒张功能相关参数。比较高血压组与正常对照组的PP和左室功能。高血压组分为高PP组和低PP组,对比分析两组左室功能和结构。结果高血压患者脉压(PP)和心功能指标(EF、EPFV、APFV、E/A)较正常对照组差异有统计学意义(P<0.01或P<0.05)。高血压组患者血压参数与年龄、体重指数及心功能参数作相关分析,示PP与年龄正相关,PP与二尖瓣E峰及E/A值负相关(P<0.01)。高血压患者高PP组左室舒张功能不全及左室肥厚率明显高于低PP组(P<0.01),而收缩功能不全的发生率两组均较低,比较差异无统计学意义(P>0.05)。结论高血压患者的PP主要与左室舒张功能有关,高PP者左室舒张功能不全发生率高。 Objective To investigate the relationship between pulse pressure (PP) and left ventricular function in patients with essential hypertension. Methods Blood pressure and left ventricular function were observed by sphygmomanometer and color Doppler echocardiogram in 92 patients with essential hypertension.53 healthy persons were taken as control group. the 92 patients with hypertension were divided into two groups: high PP group and lower PP group . Results There was significant difference in pulse pressure (PP) and left ventricular function parameters (EF,EPFV,APFV,E / A) between patients with hypertension and normal persons(P<0.01 or P<0.05=.Linear correlation analysis showed that PP was negatively correlated with E peak and E / A ratio. There was no correlation between PP and left ventricular systolic function indexes. Compared with lower PP group, the high PP group showed higher incidence of left ventricular diastolic dysfunction(P<0.01=. Conclusion The increased PP has influence on left ventricular diastolic function in patients with essential hypertension ; the wider PP is associated with a higher rate of left ventricular dysfunction.
