摘要
目的探讨周围型肺癌的CT征象与肿瘤血管内皮生长因子(VEGF)和微血管密度(MVD)表达的关系。方法对32例经手术病理证实的周围型肺癌(腺癌17例,鳞癌15例)采用LDP免疫组化法,检测肿瘤标本中VEGF和MVD的表达,并分析其与术前CT征象的关系。结果VEGF与MVD值呈正相关关系(r=0 6 91,P <0 0 1) ,VEGF和MVD表达与周围型肺癌的组织学类型无关(P >0 0 5 ) ,与肿瘤的大小、分叶征、棘状突起、胸膜凹陷征、血管集束征、纵隔淋巴结转移及胸腔积液等CT征象均有关(P <0 0 5 )而与毛刺征无关(P >0 0 5 )。结论周围型肺癌的部分CT征象与VEGF和MVD表达密切相关,当肿瘤直径>3cm、或出现分叶征、棘状突起、胸膜凹陷征、血管集束征、纵隔淋巴结转移及胸腔积液等征象,提示肿瘤可能有较高的恶性程度。
Objective:To investigate the relationship between CT signs and the vascular endothelial growth factor (VEGF) and the microvessel density (MVD) in peripheral lung cancer.Methods:The expression of VEGF and MVD was estimated with LDP immunohistochemical technique in 33 cases (adenocarcinoma 17 cases and squamous cell carcinoma 15 cases) with surgery and pathologically proved peripheral lung cancer.The ralation between preoperative CT signs,the VEGF and the MVD was retrospectively analyzed.Results:The expression of VEGF had positive correlations to their corresponding MVD values (r=0.691,P<0.01).The VEGF and the MVD value carried no relation with the type of lung cancer (P>0.05).The VEGF and the MVD value were related to the lesion's size and lobulation sign, spinous protuberance, pleural retraction sign,vessel convergence sign,lymph node metastasis and pleural effusion (P<0.05),though it showed no relation to spiculation sign (P>0.05).Conclusion:The CT signs of peripheral lung cancer are closely related to their VEGF and MVD values. High malignancy is suggested for peripheral lung cancer when the diameter of tumor is greater 3cm,or CT signs, lobulation sign, spinous protuberance, pleural retraction sign,vessel convergence sign,lymph node metastasis and pleural effusion, are present.
