The evaluation of the vein valve function in lower limbs by color Doppler ultrasound

摘要 目的探讨二维超声及彩色多普勒血流显像(CFFI)技术在下肢静脉瓣膜功能检查中的应用价值。方法对4 0例患者5 4条下肢和2 7例正常对照组5 4条下肢的股深、股浅静脉和静脉内径、瓣膜返流程度、返流持续时间进行测定。股静脉瓣膜功能做Valsalva试验,静脉瓣膜功能做远端小腿挤压试验。结果病变组二维超声显示静脉管腔内径增宽大于伴行动脉;瓣膜回声增强;可有侧枝循环形成。正常对照组返流持续时间<0 5s。静脉造影对照显示,病变组瓣膜返流时间较正常组显著延长(P <0 0 1)。返流0级10条,返流Ⅰ级2 1条,返流Ⅱ级10条,返流Ⅲ级7条,返流Ⅳ级6条。结论超声检测下肢静脉瓣膜功能可准确定位血管病变段、静脉返流程度及返流持续时间,对临床诊断和治疗方案的选择有实用价值。 Objective:To probe into the application of 2-D ultrasonography and color Doppler in the examination of lower limbs vein valves.Methods:The depth of femoral, shallow vein of femoral, inside path of the vein and the extent of valve reflux duration in 54 lower limbs of 40 cases in subject group and 54 limbs of 27 in control group were measured. The Valsalva experiment of femoral vein valve was carried out. The distant small legs were comppressed to test vein valve function.Results:The increase of the 2-D vein internal diameter in subject group was larger than that of vascularity; the resonance of valve was intensified; the collateral circulation was formed. The duration of reflux in control group <0.5s.In contrast with vein imaging, the duration of reflux in subject group was noticeably extended (P<0.01). In subject group the degrees of the valve reflux are respectively 10 cases in reflux 0, 21 cases in reflux Ⅰ, 10 cases in reflux Ⅱ, 7 cases in reflux Ⅲ, 6 cases in reflux Ⅳ.Conclusion:The use of ultrasound in lower limbs vein valve can accurately fix the position of blood vessel lesion, the degree of vein reflux and the duration of reflux. This technique has its application value in diagnosis, the choice of therapy and positioning of surgery.
