摘要
目的 :探讨二维、三维超声心动图 (3DE)检测扩张型心肌病左室心肌重量及收缩功能的诊断价值及相关性。方法 :扩张型心肌病病人 32例 ,采用 HP5 5 0 0彩色多普勒超声心动图仪采集其二维图像 ,然后在 Tom Tec echo- scan三维超声工作站脱机进行三维超声图像重建 ,采用三维圆盘总和法测量患者左室心肌重量 (L VM)和左室舒张末期容积 (L VEDV)、左室收缩末期容积 (L VESDV)和射血分数 (EF) ,同时应用常规二维超声心动图 (2 DE) Simpson法测量上述指标。结果 :32例扩张型心肌病患者中 ,2 7例获得满意三维重建图像。应用 3DE检测 L VEDV为 (16 5 .15± 2 6 .5 9) m l,与二维测量的结果 (15 8.4 4± 2 2 .4 9) m l相关性良好 (r =0 .75 4 ,P <0 .0 1) ,3DE测量 L VESV为 (12 2 .74± 2 7.13) ml,与 2 DE测量结果 (115 .96± 2 2 .5 6 ) m l呈高度正相关 (r =0 .837,P <0 .0 1) ,3DE测量 EF为 (0 .2 6± 0 .0 5 ) ,与 2 DE测量结果 (0 .2 7± 0 .0 5 )呈高度正相关 (r =0 .875 ,P <0 .0 1) ,3DE测量 L VM为 (30 2 .74± 31.72 ) g,与 2 DE测量结果 (2 78.0 7± 39.0 2 ) g相关性良好 (r =0 .6 91,P <0 .0 1)。 3DE检测 L VM与 EF呈高度负相关 (r =- 0 .897,P <0 .0 1)。结论 :二维、三维超声心动图均可用于测量?
Objective:The aim of this study was to detect the diagnostic values and correlativity of two and three dimensional echocardiography (3DE) on dilated cardiomyopathy.Methods:32 cases were studied randomly. Two dimensional echocardiography was performed in 32 patients by using HP/SONOS 5500 ultrasound scanner and 3DE was reconstructed on TomTec echo scan station . The left ventricular mass (LVM), left ventricular end-systolic volume (LVESV) and end-diastolic volume (LVEDV) were measured in 27 patients from each three dimensional echocardiographic data set with the “disk summation” method. Ejection fraction (EF) was then derived. All above data were also measured by using Simpson method from two dimensional echocardiography (2DE).Result:The satisfied 3DE images were acquired in 27 patients of 32 patients. The LVEDV, LVESV, LVEF and LVM from 3DE in 27 patients showed excellent correlation with those measured from Simpson method [(165.15±26.59) ml VS (158.44±22.49) ml,(122.74±27.13) ml VS (115.96±22.56) ml, (0.26±0.05) VS (0.27±0.05), (302.74±31.72)g VS (278.07±39.02)g, respectively, r=0.754, 0.837, 0.875 and 0.691, respectively, P<0.01, respectively]. The LVM showed excellent correlation to LVEF from 3DE(r=-0.897, P<0.01).Conclusion:Both 2DE and 3DE can be used to measure left ventricular volume, systolic function and LVM for DCM with an excellent correlation.
