摘要
恶性脑胶质瘤治疗困难,死亡率高。近十年来发展的大剂量化疗联合外周造血干细胞移植治疗恶性脑胶质瘤取得了一些疗效,特别对髓母细胞瘤、原发性神经外胚层肿瘤和高级别的星型细胞瘤有较好的效果。目前所用的治疗方案,大部分是单一的大剂量双氯乙基亚硝脲的治疗方案(800-1000 mg/m2),另有一些多药组合方案,但大剂量方案引起了较多的不良反应,而并未显著提高患者的平均生存时间,故仍需新的药物和方案进行临床试验。
High-grade malignant gliomas are inevitably fatal, despite every effort to improve their prognosis. The use of high-dose chemotherapy with stem cell rescue for malignant brain tumors is reviewed. Promising results have been reported in patients with medulloblastomas, supratentorial PNET's and high-grade astrocytomas. The most frequently used drug in the conditioning regimens is BCNU at doses of 800-1000 mg/ m2. These dosages involve a high toxicity that is not balanced by a significant improvement in survival. New drugs and/or regimens should be tested in randomized trials.
