摘要
目的 探讨经粒细胞集落刺激因子(G CSF)动员后自体外周血干细胞移植(PBSCT)对兔心肌梗死心功能的影响。方法 4 0只日本大耳白兔随机分成两组,每组2 0只,均结扎冠状动脉前降支建立心肌梗死模型,两组均给予G CSF连续7天,于模型建立后1周,将分离纯化的外周血单个核细胞悬液经心外膜注射到心梗区及周边,而对照组只给予等量的生理盐水,在术后4周测定两组梗死面积的大小、毛细血管密度以及纤维化的情况,并观察植入细胞的存活及分化情况。于术前、术后2 4h及术后4周做心脏超声检测心功能。结果 移植组毛细血管密度大大增加(P <0 .0 1) ,梗死面积较对照组明显减小;Masson染色显示对照组心肌纤维排列紊乱不规则,而移植组排列有序,且蓝色的胶原纤维明显少于对照组。心功能各项指标在术后4周时均较对照组有明显的改善,射血分数增加了15 %。结论 经G CSF动员后自体外周血干细胞移植可显著改善心肌梗死兔的心功能。
Objective To study the effect of transplanting autologous peripheral blood stem cells on cardiac function of rabbit of myocardial infarction(MI).Methods Japanese big-ear rabbits,MI model was developed by ligating the anterior descending coronary artery.Granulocyte colony-stimulating factor(G-CSFs) were given continual 7 days in both groups.Segregated and purified cell suspensions were injected into infarction regions one week after setting up of the model,while in control groups only the same doses salines were injected.We detected the size of infarcted focus,capillary densities and fibrosis and detected the survival and differentiation of the implanted cells.Cardiac functions were tested by echocardiography before as well as 24 hours and 4 weeks after operation.Results In transplantation groups there were a large amount of neogenesis capillaries (P<0.01),moreover the infracted areas were greatly decreased.Masson staining showed the structures of infarction regions were deranged in control group,but in transplantation groups cell arrangements were in order,meanwhile in control group blue collagen fibers were more than in transplantation group.The cardiac functions were much better than in the control group at 4 weeks.Ejection fraction was increased 15%.Conclusion Transplanting autologous peripheral blood stem cells mobilized by G-CSF can obviously improve cardiac function of rabbit MI.
