摘要 目的: 探讨人细小病毒B19感染与小儿川崎病的关系.方法: 用巢式PCR法对46例川崎病患儿进行B19 DNA检测,用ELISA法对其中的30例患儿进行B19 VP2 IgM检测.结果: 病例组46例,B19 DNA阳性14例( 30% ),对照组50例B19 DNA阳性2例,两组间差异显著(P<0 01).病例组B19 VP2 IgM阳性7 /30例,对照组50例均阴性.两组间差异极显著(P<0 01). 30例川崎病患者中B19 DNA,B19 VP2 IgM均阳性6例; 1例仅B19 VP2 IgM阳性; 4例仅B19 DNA阳性;B19 DNA和B19 VP2 IgM同时阴性19例,B19 DNA和B19 VP2 IgM一致率为83 3%,有一致性(P<0 01 ).B19阳性与阴性川崎病患儿在性别、年龄、常见临床表现、预后等方面无差别.结论: 我国川崎病患儿B19病毒感染率较高可能是导致川崎病的主要病原体之一. AIM: To study the relationship between human parvovirus B19 infection and Kawasaki disease in children.METHODS: Nested PCR was used to detect serum B19-DNA in 46 children with Kawasaki disease and ELISA was used to detect serum B19-VP2-IgM in 30 patients.Fifty healthy children were used as control.RESULTS: Fourteen(30.4%) of the 46 patients in the case group and 2(4.0%) of the 50 healthy children in the control group were positive for B19-DNA(P<0.01).Seven (23.3%) of the 30 patients in the case group and none of the children in the control group were positive for B19-VP2-IgM (P<0.01). Of the 30 patients in the case group whose B19-DNA and B19-VP2-IgM were tested,6 were positive for both B19-DNA and B19-VP2-IgM,while another 4 were only positive for B19-DNA and 1 for B19-VP2-IgM,and 19 were negative for both B19-DNA and B19-VP2-IgM.Crude agreement between the detecting results of B19-DNA and B19-VP2-IgM was 83.3% and they were significantly consistent(P<0.01).There was no difference in sex,age,clinical manifestation and prognosis between the patients with positive B19 and those with negative B19.CONCLUSION: Parvovirus B19 infection rate of Chinese children with Kawasaki disease is high,which may be one of the main pathogenic agents leading to Kawasaki disease.
